Colorado Immunization Information System (CIIS) Notice

Our hospital participates in the Colorado Immunization Information System (CIIS), a confidential statewide immunization registry maintained by the Colorado Department of Public Health and Environment.

Information about vaccinations administered at our facility may be submitted to CIIS as permitted by Colorado law. Patients, parents, and legal guardians have the right to request that immunization information be excluded from CIIS (opt out).

If you choose to opt out, you will be responsible for maintaining your own immunization records. You may also rescind an opt-out request at any time.
