2025-26 Updated Materials Available

Updated documents for the 2025-26 fiscal year are now available on the Hospital Discounted Care website as outlined below. All documents have removed references to the Colorado Indigent Care Program (CICP) and have additional changes as noted. Please ensure these versions are used beginning July 1, 2025 for all patients. Updated rate files for 2025-26 are also available on the Hospital Discounted Care Rates website. Questions can be sent to hcpf_HospDiscountCare@state.co.us.

Uniform Application

Both the Excel and PDF versions of the Uniform Application have been updated. Providers must update the application they have posted on their website regardless of whether they post the PDF or Excel version to meet posting requirements.

The Excel version of the Uniform Application has been updated with the following changes:

●All CICP references and CICP Only information has been removed completely

●Insurance question on the Screening Form tab has been changed to "Do you have insurance?" from "Are you uninsured [or are you about to lose your health insurance]?"

●Medicaid ID question on the Screening Form tab has been changed to "If so, what is your Member ID if you know or have it?" from "If so, do you have or know your ID number?"

●Social Security income line has been updated to remove "SSI", as Social Security income is counted, not Supplemental Security Income

●Unemployment Income calculator on Worksheet 1 has been updated to not include unemployment income in the calculation if the calculated last payment is before the date of the application

●Worksheet 2 has been updated to allow for two separate businesses to be entered and has new checks to ensure information is being entered correctly

●Application tab has new options for what the household member has been approved for, including Hospital Discounted Care, Charity Care, Hosp Disc Care/Charity Care, and HH Size Only

●Card Template has been updated to list household members approved for Hospital Discounted Care and household members approved for the hospital's Internal Charity Care

●Hospitals can enter their internal charity care program name on the Card Template tab and the name will reflect on the card

HCPF continues to accept suggestions on how to improve the Uniform Application, and suggestions may always be sent to hcpf_HospDiscountCare@state.co.us.

Patient Rights

The Patient Rights document was discussed at the April CICP Advisory Council meeting and has been updated with comments and suggestions from stakeholders. Additional changes were made through HCPF's internal communications review process to ensure the document is written at a sixth grade reading level as mandated by statute. The updated document is available on the Hospital Discounted Care website in both English and Spanish under the Forms blue bar. Providers must use the updated version beginning July 1, 2025.

Decline Screening Form

The Decline Screening Form has been updated to remove the reference to the CICP. The updated document is available on the Hospital Discounted Care website in both English and Spanish under the Forms blue bar. Providers must use the updated version beginning July 1, 2025.

Sample Determination Notices

HCPF has updated the sample determination notices to remove references to the CICP. The updated document is available on the Hospital Discounted Care website under the Forms blue bar.
